please add a menu entry to start GRASS with the wxPython GUI.

Using the latest 6.4.0svn-2 package I've got Start -> All Programs -> OSGeo4W -> GRASS GIS -> | Text | wxPython

or do you mean in the "Express Install" section of the installer itself?

After a update today, these are present. So can be regarded as fixed. The icon left from the old versions

Start -> All Programs -> OSGeo4W -> GRASS

hasn't been removed, though.
