Description
Folder entries are created for each layer requested. Under each folder should be a CLASS Folder (if NAME included) and entries for each LABELITEMed PlaceMark

Class folders won't be possible with the current architecture AFAIK; class info is not available at low rendering level. Something to look after the first cut. Items are added in the main folder of the layer.
