csv2html and cpl_serv.h instead of cpl_csv.h

Description
Should csv2html use cpl_serv.h instead of cpl_csv.h? As part of my automake work (I have it almost all done except for some tiff configuration stuff, btw), I noticed that I needed to change the include in csv2html.c. Should this be changed?

csv2html is really not relavent anymore as lots of other options exist to view EPSG parameters. I am removing it (r2028). I also suspect it wasn't up to date with the current schemas.
