Update disclaimer documentation

The disclaimer documentation at ​http://geomoose.org/extensions/Disclaimer.html has a couple issues

1) The line "To add Dynamic Select to your application add:" should be removed

2) For 2.2 users the disclaimer string is set in the html file instead of within disclaimer.js. The documentation should inform 2.2 users to update disclaimer.js (so everything is uniform), or inform users that DISCLAIMER_MESSAGE must be set.

3) At 2.2 Disclaimer.js has a missing "}" in the if then else statement.

2) We are currently not maintaining 2.2 docs so there is nothing to do
3) Jim addressed this on the email list (2.4 is mostly the continuation of 2.2 and the fix is in the 2.4 line).
