Missing FROM column for a JOIN causes a segfault

Description 

If the FROM column in a JOIN is missing from the dataset then MapServer
segfaults. I did not test the TO column. Instead MapServer should fail gracefully.

Fixed in CVS head, was only an error with URL templates.
