shp2pgsql attribute names mapping -m switch

Description

Since introduction of -m switch in pgsql2shp (ticket #885) it would be nice to have an -m switch in shp2pgsql, for simmetry.

To be extra nice, the _same_ mapping file should be used, which means reading DBF names on the second column and db attribute names on the first column.

Committed at r12821 for PostGIS 2.2. (includes regress test and doc update) Will close once bots have confirmed all is fine.

doesn't seem I can win here. Native mode didn't seem to work. Will try that again.

windows line breaks worked fine on my machine and on winnie but debbie complained. Changing line breaks to linux ones of mapping file at r12823 calmed debbie down but made winnie unhappy.

hmm I guess both debbie and winnie are happy with linux line breaks. I think. I'll close now while the bots are both happy.

Travis is still working but his last complaint was totally illogical.
