"Create spatial database" not working in Windows installer

Description
I installed PostgreSQL 9.4 x64 from http://www.enterprisedb.com/products-services-training/pgdownload#windows and then in Stack Builder installed the PostGIS 2.1 bundle. I selected "Create spatial database", entered credentials, typed "template_postgis" as DB name, pressed YES on all prompts, but the database was NOT created, I checked with pgAdmin.
End of install log:
