Missed icons for some GdalTools utilities (QGIS)

Description
Several GdalTools utilities in QGIS don't have icons:
- RGB to PCT - Convert a 24bit RGB image to 8bit paletted
- PCT to RGB - Convert an 8bit paletted image to 24bit RGB
- DEM (Terrain models) - Tools to analyze and visualize DEMs
- Tileindex - Build a MapServer raster tileindex
Also there is an new tool under development, which is currently also without icon:
- Fill NoData - Interpolate in nodata regions
It would be nice to have icons for all this tools. There is no urgency, of course.

First two icons for RGB and PCT are ready.
