Add existing MS4W Packages

Description
I'll hit Gmap first (because Assefa added a Fusion package yesterday that requires it), then the MapServer Itasca Demo, OpenLayers, Chameleon, and ka-Map. For the other packages, I will contact the people who originally packaged them for ms4w, and if I don't get any commitment to maintain an osgeo4w package from them that package will not be created (since the goal of osgeo4w is to get people involved from the individual communities as package maintainers).

- gmap package added to server
- parts of this package rely on custom epsg codes (ticket#228)
- php_mapscript package added to server
- modifications done to php package for gmap
- tested fusion package with gmap package from telascience server...works!

- added mapserver demo and chameleon packages
- tested locally and from the osgeo download server
notes:
- had to modify javascript in the 5.0 mapserver itasca demo because the backslash in the osgeo4w path (e.g. C:\OSGeo4W) was causing trouble
- chameleon package includes the required /tmp/ms_tmp/buttons/ folder.

- added openlayers package
- removed odd temporary files ("~") from the ms_ogc_workshop package, and uploaded new package (ms_ogc_workshop-1.0.9-2.tar.bz2)
- added package versions to the ms_ogc_workshop and fusion packages, per the OSGeo4W packaging guidelines (e.g. ms_ogc_workshop-1.0.9.tar.bz2 becomes ms_ogc_workshop-1.0.9-1.tar.bz2)
- that change may cause a few of you to have a 'corrupt' message from the installer, if u already have one of those packages, so just delete your temp packaging folder (sorry!)

I have contacted the maintainers of the MS4W packages, that aren't already in OSGeo4W, and asked them to contribute to OSGeo4W (Brian Fischer for GeoMOOSE, Aaron Koning for FIST, Ken Boss & Steve Lime for dBox, and Eduin Carillo for ArcIMS Emulator). p.mapper's maintainer no longer works in the spatial industry so that package is in limbo, so hopefully someone from that community steps up (I don't even know p.mapper and i've been the one making p.mapper packages).
