WMS Server Documents needs more info

http://mapserver.org/ogc/wms_server.html needs to be updated to reflect:

a) template files must have an .html extension
b) template files musst have the first line of <!-- MapServer Template -->
c) if you are using info_format=text/html on a GetFeatureInfo request, then you must have template files to generate the appropriate document
d) it is not clear how to get a feature's geometry returned in the gml of a GetFeatureInfo request

A fix has been attempted for trunk and 6.0 documentation in r12597. References to the template document have been included. gml_geometries and gml_[name]_type mentioned.
