grey out layer based on scale dependency

Description

This patch allows catalog layers to be greyed out at certain scales. Two additional parameters on the <layer> mapbook tag configure the max and min scales, respectively. These options are meant to be used with their equivalents in the layer definition in the mapserver mapfile.
