toolbars have no buffer around icons with Qt 4.6

Description
With Qt 4.6 on OSX, both Carbon 32bit and Cocoa 64bit, there is no buffer around icons in toolbars, except the OSM toolbar. In the attached image, all toolbars are like the GRASS toolbar.

It looks like it was a Qt problem. Problem reported with Qt 4.6.1. I just updated to Qt 4.6.2 and toolbar icons have a buffer around them again.
