WCS 1.0 rangeset_axes parsing crashes in some cases

Description
If a WCS 1.0 describecoverage request is made when the mapfile contains the wcs_rangeset_axes metadata but it does not contain "bands" then mapserver will crash.

This appears to be a very old error, but has not come up before because it is very rare for us to define other axes. I have corrected the bug in trunk (r12543) and 6.0 branch (r12544).
Ideally we would have an autotest for this but I am not willing to take the time now to construct one.
