Drop map-specific query modes for the MapServer CGI

Description

Need to drop the map-specific query modes from the MapServer CGI. They're unnecessary and I'd rather promote the use of the qformat attribute instead.

Committed in r11078. Now moving to a documentation task. Need to note the modes that are no longer supported but also make sure qformat is adequately described.

Agreed, I updated it in r11109. That document actually needs a lot of work before we release. I'll take a pass at it and make sure things related to my stuff (e.g. expression parsing, label styles, and MapScript? query processing) are documented.

We may want to consider something beyond a text file for this purpose. I think Astrid was creating a Wiki page for just such a purpose.

All mentions of *QUERYMAP have been removed from the 6.0 and trunk documentation in r12576. The only remains I found were in cgi-> controls, and there were very few (two mentions of NQUERYMAP and one mention of ITEMQUERYMAP).

The QFORMAT parameter was documented and a note on formats that are no longer supported was added in r12578.
