default onlineresource is wrong

When I do not specify a wms_onlineresource, mapserver kindly generates one for
me. Unfortunately, the one it generates is not right! It is the same both in the
top level and in the GetMap, GetCapabilites sections.

It should be:

http://mapserver.refractions.net/cgi-bin/mapserv?map=/home/www/mapserv/maps/postgis.map&

It comes out as:

http://mapserver.refractions.net/cgi-bin/mapserv?

Since the WMS instance is slaved to the map file, the map file reference has to
be there. Many clients (CadCorp, for example) read the capabilities, and then
use the onlineresource from from the capabilities to procede to the GetMap, etc.
