About your last question, my understanding is that in WMS 1.3, any use of "regular" epsg codes (e.g. EPSG:4326) requires that we use the axis order from the EPSG database, which is the reverse of what we used to do in WMS 1.1 and older... and also doesn't match what you say our WCS implementation is doing.
