Using MS for Windows, CLASS TITLE is not applied to label items in the legend. NAME is used even when TITLE is present.

TITLE isn't used anywhere as far as I know. At one point (still do in fact) I regreted not having the ability to refer to classes using a short name (like we have with layers) and I toyed with idea of using TITLE for legends and NAME for identification of a class. That never happened though, not completely, and it certainly isn't documented as working (Russell, how did you come to try this?).
I'd be tempted then to leave this bug, but mark it for a FUTURE milestone.

FYI I don't see CLASS.TITLE as a documented directive of the CLASS object:
http://mapserver.gis.umn.edu/docs/reference/mapfile/class
...so this might be why it's not being used/displayed.
Is there any particular use case which would require both NAME and TITLE in the CLASS object?
