Update WKB Reader / Writer API docs to be more clear about specification

Description
Presently, for non-2D geometries, the GEOS WKB Reader / Writers only handle Extended Well-known Binary (EWKB), described through PostGIS's specification http://trac.osgeo.org/postgis/browser/trunk/doc/ZMSgeoms.txt
OGC's implementation of 3D (Z/M) and 4D (ZM) geometries are presently not supported by GEOS, but this isn't clear. The documentation only makes sense from the perspective of OGC's 2D-only implementation of WKB, and the API docs should be updated to be more clear of which specifications are actually used.

Going to punt this forward if only as a reminder that we need any documentation at all. Like a real web page with some examples and getting started information instead of a scrufty trac wiki.
