MapFishPrintProxy - use 'app' parameter in createURL requests — at Initial Version

Description
With MapFishPrint, you can specify a config.yaml in a print request using the 'app' parameter. When visiting info.json, it's there as an argument returned.
In GeoPrisma MapFishPrintProxy, that argument is currently not used. Make the according changes to use it.
