vmap0 layer imports......

When you use ogr2ogr to import vmap0 layers into PostGIS then there is no option to append layers to the existing layer if available and create when it's not available.

A option like -createifnolayer (or a better name?) would be highly appriciated.

This will save a lot of scripting to import all 4 vmap0 layers into postgresql in a single run...

-append will now create the layer if it does not already exist.

This change should appear in the next FWTools release (1.0.0a7 I think) 
later today.
