Update MapServer to be able to deal with non-square cellsizes.

Description 

WMT can request maps with non-square cell sizes. Need to update the code to 
flag that condition. Scale and cellsize computations are affected. Probably 
need to create cellsizex/y and scalexy variables instead of the single value 
setup that is currently in place. No biggie.

I think this should be considered seriously in 4.1... depending on how much work 
would be involved of course.
