.bt bugs on write

It is indeed deathly slow. However I could live with that if it allowed me to
automate conversion of our AI grids to VT. Unfortunately it still doesn't work
(as of gdalcvs-040203.zip). A sample dataset is attached which holds both my
source data and the results.

Ben is this something you have some insight into? Loading an ArcInfo Binary Grid
into VTBuilder and saving it out as .bt doesn't take all that long.

Not so much that you'll pull it out altogether I hope! (but then again I guess
if it doesn't work there's not much point in keeping it around.)

All problems corrected with what is essentially a rewrite of the driver.
